Rhythm Pharmaceuticals is a high-growth commercial-stage company targeting orphan neuroendocrine disorders linked to MC4R pathway dysfunction (genetic obesity). Having expanded from one disorder to multiple approved indications, the company has additional opportunities ahead to expand its portfolio. I rate RYTM a BUY, driven by expected double-digit revenue growth, a broader targeted-obesity portfolio, and stronger-than-expected patient uptake.

Anorexigenic neurons activate POMC, which PCSK1 cleaves into peptides including α-MSH. α-MSH activates MC4R to suppress appetite and increase satiety and energy expenditure. Diseases such as Bardet-Biedl syndrome and POMC/LEPR deficiency impair MC4R activation by reducing α-MSH production, causing severe hyperphagia (feelings of extreme hunger). IMCIVREE addresses this by reducing hyperphagia and increasing energy.

Q2 2026 Results:

Management reported strong early AHO uptake for IMCIVREE, with 400+ start forms from 300 prescribers, ahead of model assumptions. Broad prescribing suggests adoption beyond specialty centers, while international patient growth exceeded 20%. Japan AHO approval is expected before year-end, adding 6,000–8,000 addressable patients.

M&A Potential:

Rare-disease/obesity-adjacent M&A has been active and richly priced: Crinetics Pharmaceuticals' endocrine pipeline was valued at a reported $10bn, and Soleno Therapeutics (a direct PWS competitor) was acquired for approximately $2.9bn. These transactions suggest strategic and financial buyers are willing to pay substantial premiums for de-risked rare endocrine/obesity assets — a dynamic that could support multiples above our SOTP assumptions if RYTM itself became a takeout candidate. Consequently, it is our view that M&A potential increases as management further executes its clinical program.
